For the convenience of our clients we have included some of the forms that need to be filled out upon arrival at the office. Feel free to print out the forms and so your questions can be answers thoroughly.

When you are making the copy for Medical Authorization you will need to make a copy for each person that we are going to release information to. That includes Probation &/or Parole officers, Lawyers, And Medical Professionals. (Please include their phone number & fax numbers.)
